Proposal Request
Having detailed information is critical in the process of obtaining both a competitive and credible proposal for the group. Below is a list of information that is necessary to get started on a health proposal.
- Employer name, address, and industry
- Requested effective date
- Current stop-loss deductible and desired new deductible options
- Current contract rates and type for both specific and aggregate
- Two years claims experience, by month
- Two years shock-loss claims experience including diagnosis and prognosis
- Current plan design and any proposed changes
- Census that includes: DOB, gender, and tier
